OUR POLICIES

Until further notice, all patients and parents over the age of 2 years are required to wear a mask that covers mouth and nose. If you are coming to the clinic for a sick visit, please call the office when you arrive and are in the parking lot.

Hasta nuevo aviso, todos los pacientes y padres mayores de 2 años deben usar una máscara que cubra la boca y la nariz. Si viene a la clínica para una visita por enfermedad, llame a la oficina cuando llegue y esté en el estacionamiento.

At Healthy Days, we want your visit to be as streamlined as possible. We also want the process to be fair for everyone.

Please take a look at our general policies so that you can be prepared prior to your visit and there will be no surprises.

Scheduling:

Our office is open from 8am–5pm; appointments are scheduled from8:30–3:30pm, with a break for lunch from 12–1pm.
Appointments for physicals or vaccines
- Can be scheduled as far ahead as 3 months.
Sick Visits will be scheduled as soon as possible and generally within 24
hrs
Same Day appointments:
- Will be scheduled starting at 8 am and callers will be placed on awaiting list.
- Sicker or younger patients will be called in first off
  waiting
  list
- Should the waiting list be filled when you call, your child will be
  placed on the waiting list for the following day
In general, we do not accept walk-ins. If you walk-in without an
appointment, your name will be added to next slot on the waiting list
Vaccine only visits: We provide vaccines without a physical only if
Healthy Days Pediatrics has a documented physical exam within the last 6 months.

Insurances:

We currently accept Traditional Medicaid, Superior, El Paso First, Blue
Cross Blue Shield, Cigna, United Health, Molina, Aetna and
Tricare Insurances.
Copays
- Will be collected at time of visit.
- Copay amounts are determined by your insurance plan.
Self-Pay
- We gladly accept self-pay patients; payment is expected at time of visit.
- Please inform us immediately if you think you will be unable to afford
  the visit and we will attempt to make arrangements to assist you

School Forms and Vaccination Cards:

Please fill out the student section of the sports physical form (see
below) and bring both pages to your visit
Please bring any Head Start, school nurse, school cafeteria or other forms
with you if they need to be completed
Please bring your vaccination card to each visit. Verification of
immunization status is required.

Letters and Prescription refills:

For refills, please call at least 5 days before you run out of medicine
ADHD prescriptions cannot be called into a pharmacy. A parent must come to
pick up the prescription in person.
Letters for IRS, INS, judges/lawyers, schools or employers will require
the following:
- full name, title and address of individual or organization
- a separate signed release of information form
- reason for letter
- at least 10 days between initial request and date it is needed

Other Info:

Timeliness: If you are more than 15 minutes late for your appointment, you
may need to reschedule. Please call us if you know you are going to be late.
No shows/Cancellations: if you are unable to keep your appointment, please
call us 24 hours in advance to cancel or reschedule. A $25 fee may be charged for if you do not keep your
appointment
We kindly request that cell phones be turned off when you are with the
physician
We kindly request that you not bring food or drinks into the building to
protect your children from spills or pests. We do allow baby formula and water.
